We are expanding our clinical capacity and looking for a highly motivated and enthusiastic part time Diabetic Practice Nurse to join our friendly dedicated team at Fountain Medical Centre. It is essential you have a Diabetic Diploma. The ideal candidate should have excellent people, planning and organisation skills and able to manage your own workload.
The successful candidate will work alongside a multidisciplinary team made up of GP's, Advanced Nurse Practitioners, Clinical Pharmacists and Nursing team to provide treatment and care to patients. Support, supervision and mentoring will be provided. Main duties of the job
Diabetic Practice Nurse experience is required. Other training will be supported and offered. Some of the key duties will include:
Chronic Health reviews and management
Cytology
Wound care/Dressing management
Travel Clinic
Routine immunisations/Childhoodimmunisations
Following agreed clinical protocols withreferral to senior nurse or GPs as appropriate
Must be a Registered General Nurse and holdfull registration with the NMC. Diabetic diploma required and other diplomas are essential.
Hours are Tuesday 12:00-4:00pm / Wednesday 8:00-6:00pm / Thursday 8:00-6:00pm / Friday 8:00-6:00pm

Person Specification
Qualifications
* Registered first level nurse
* Minimum 3 years post registration experience
* At least 2 years recent primary and community nursing experience
* Relevant qualifications for nurse led clinics
* Knowledge of management of patients with long-term conditions
* Clinical skills - immunisation & vaccination, Cytology, sexual health, ear care and chronic disease nurse led clinic
* Knowledge of PGD and associated policy
* Knowledge of health promotion strategies and of local and national policies
* Experience of quality initiatives such as clinical benchmarking
* Knowledge and experience of working in primary care in a GP setting
* Independent/supplementary nurse prescribing qualification
