Provide individual and accommodation-based support to single clients accommodated at Cedar Lodge at Stanstead Abbotts or any similar temporary accommodation, for previously homeless or rough sleeping applicants, who will likely have support needs including, mental health, drug and alcohol use, history of criminal/anti-social behaviour, or long-term unemployment. Carrying out outreach tasks to verify rough sleeper across East Herts Borough and supporting verified rough sleepers to ensure they have an identified housing solution that meets their needs and supporting them to achieve and sustain them. Listen to and build rapport with the residents to encourage positive engagement with the service. Develop support plans with agreed achievable outcomes in collaboration with the resident at cedar cottage (single client accommodation), relevant support agencies and council officers, notably the Housing Options Officers Ensure that the required case files and paperwork are kept up-to-date and regularly monitored and contribute to the provision of timely and accurate information as required to manage the service.
