Overview
An exciting part-time opportunity (30 hours) for a Senior Registered Nurse to join our Community Mental Health Team based at Worsley Court, Selby, to support older adults living with mental health needs. The service offers assessment and treatment within the community setting with the aim of promoting recovery and living well. The post is suitable for an experienced practitioner who wishes to develop leadership and clinical skills in a community setting and work as part of a multi-disciplinary team.
As a core member of the team, you will manage your own caseload, providing assessments, care coordination and delivering intervention in the community for people with complex needs ranging from mild to severe and enduring levels of need. You will demonstrate excellent assessment, communication, and teamwork skills and work collaboratively with a range of statutory and third-sector agencies.
Responsibilities
* Lead as a senior practitioner within the Community Mental Health Team for older adults.
* Manage a defined caseload: carry out comprehensive assessments, develop care plans and provide care coordination.
* Deliver evidence-based interventions in the community and support people with complex needs.
* Participate in and contribute to service development, including managing emotions and carers support groups alongside existing services.
* Collaborate with a range of statutory and third-sector agencies to enable holistic care.
* Travel independently across the locality and have access to a suitable vehicle for business purposes (adjustments under the Equality Act 2010 can be considered).
Qualifications and Skills
* Registered Nurse with current registration and a valid pin.
* Experience in community mental health care, with a focus on older adults and complex needs.
* Strong assessment, interpersonal and team-working skills; capable of working with carers and families.
* Clinical leadership capability and commitment to professional development.
Working Arrangements and Support
The team operates across two sites (including York). You will receive regular appraisal, clinical supervision and caseload supervision. You will benefit from the support of an experienced multi-disciplinary leadership team, including the Team Manager and Advanced Practitioner.
