Clinical Pharmacist – Primary Care | Reading | Excellent Training Practice
A fantastic opportunity has arisen for a Clinical Pharmacist to join a highly regarded, forward-thinking GP training practice based in Reading. The Role
The successful Clinical Pharmacist will work as part of a collaborative primary care team, playing a key role in improving patient outcomes and supporting safe, effective prescribing.
Key responsibilities include:
Conducting structured medication reviews and medicines reconciliation
Supporting patients with long-term conditions and complex medication needs
Managing repeat prescriptions and prescribing queries
Providing clinical advice to GPs, nurses, and other healthcare professionals
Supporting QOF, audits, and prescribing optimisation initiatives
Delivering patient-facing clinics (face-to-face and telephone)

Requirements
Qualified Pharmacist registered with the GPhC
Experience working in primary care or GP practice preferred (training can be provided)
Independent Prescriber qualification desirable (or willingness to work towards)
Strong clinical knowledge and patient-focused approach
Excellent communication and teamwork skills
