Support Coordinator - County Durham
Location: County Durham (Brandon, Consett, West Cornforth, Durham) and Sunderland
Hours: 37.5 hours per week on a rota, including evenings, weekends, and some waking nights.
Travel: Between services in County Durham and Sunderland. Flexibility required to meet customer needs, with occasional changes to start/finish times.
Requirements: Enhanced DBS check (paid for by us). Access to a vehicle with business insurance (mileage paid).
Role Overview
As a Support Coordinator within CRT, your typical day will involve:
1. Creating support plans and coordinating a small team of Support Workers.
2. Assisting customers with complex needs such as homelessness, substance misuse, mental health, and social isolation.
3. Supporting daily living skills like meals, shopping, budgeting, and tenancy management.
4. Carrying out risk assessments, setting goals, planning, and reviewing progress.
5. Building strong relationships with customers, providing time for chats, collaboration, and personalized support.
6. Using technology for support plans, online learning, and team collaboration.
7. Working independently, staying calm under pressure, and demonstrating resilience.
Qualifications & Skills
* Experience in person-centred support planning and guiding colleagues.
* Skilled in coordinating and assessing customer referrals.
* Passion for supporting customers to live their best lives with teamwork and attention to detail.
