To be responsible for the delivery of nursing services, working as part of the practice multidisciplinary team, delivering care within their scope of practice to the entitled patient population. The nurse practitioner will be responsible for a number of clinical areas such as health promotion, chronic disease management, health prevention, well women and well man clinics, as well as supporting the management team in reviewing clinical policies and procedures.
The following are the core responsibilities of the Practice Nurse. There may be occasions where other tasks are required; this will depend on workload and staffing levels:
1. Support the clinical team in developing individual treatment plans for chronic disease patients.
2. Identify, manage, and support patients at risk of developing long-term conditions, preventing adverse effects on their health.
3. Provide routine nursing care to patients as required, following clinical evidence, NICE guidelines, and NSF standards.
4. Provide wound care (ulcer/Doppler, etc.) to patients.
5. Offer travel medicine services.
6. Request pathology services as necessary.
7. Conduct chronic disease clinics, delivering patient care and referring to secondary or specialist care as needed.
8. Maintain accurate clinical records in accordance with legislation.
9. Ensure effective use of read codes.
10. Maintain chronic disease registers.
11. Prioritize health issues and intervene appropriately.
12. Support the team in managing clinical emergencies.
13. Recognize, assess, and refer patients with community health needs, developing strategies for effective care pathways.
14. Implement vaccination programmes for adults and children.
15. Support the clinical team with safeguarding matters, in line with local and national policies.
