Overview
This post is for a Band 6 Occupational Therapist, Physiotherapist, Nurse or Paramedic to join the Rapid Assessment & Frailty Team (RAFT) working within the emergency care settings at the Queen Elizabeth Hospital, King's Lynn.
The role is integral to supporting the delivery and development of RAFT, providing urgent and unplanned discharge planning and admission avoidance assessments across a 7‑day service.
Key Responsibilities
* Provide comprehensive specialist assessment, diagnosis and discharge care planning for patients, with an emphasis on those with highly complex multi‑co‑morbidities and/or chronic presentations.
* Conduct prompt and effective assessments within agreed standards to ensure appropriate, safe, timely and effective discharge to home or other suitable environments.
* Assist in planning, coordination and evaluation of clinical input from the team in all clinical areas covered by the Rapid Assessment & Frailty Team.
* Seek guidance from senior team members for highly complex situations and provide specialist assessment, diagnosis and discharge planning for the individual caseload.
* Maintain accurate records as an autonomous practitioner and support service development within the specialities.
* Receive regular supervision from more senior members to evaluate and develop practice, with advice, training and support available as required.
Qualifications and Experience
Essential: UK NHS experience.
Preferred: Experience as a Professional within the Acute setting.
Must be willing to work flexibly, undertake tasks traditionally allocated to specific disciplines, and enjoy working in a fast‑paced environment.
