Main duties of the job
To collaborate with the stroke therapy team in both acute and community settings, providing a flexible range of integrated, therapeutic services for the local stroke population. Responsibilities include identifying and reviewing individual needs to ensure effective, equitable, and personalized services, supporting and encouraging integrated working across multiple services and organizations, and engaging with a broad range of health and social care providers. Additionally, to contribute to the development of quality indicators and metrics to evaluate the impact of the role in supporting service users with complex stroke needs.

* Relevant degree or clinical professional experience
Experience
* Experience in a relevant caring or health profession
* Experience in staff supervision, recruitment, training, induction, and appraisal
Skills
* Compliance with HCPC standards of conduct
* Experience of inter-agency working
