NHS Greater Glasgow and Clyde (NHSGGC)

We deliver acute hospital, primary, community and mental health services to a population of over 1.15 million people, and to a wider population of 2.2 million when regional and national services are included.

The Role

* Co-ordinate the management of discharges within the Division. The department not only manages complex discharges, but also assists patients requiring minimal intervention.
* To provide support education and specialist discharge knowledge for all directorates within the Acute Services Division. This includes all grades of nursing, medical and all other members of the multi-disciplinary team. This will involve driving/travel to other sites within a defined area.
* To apply specialist knowledge to support a changing service.
* To maintain a specialist clinical caseload offering advice and hands on intervention when appropriate to patients, relatives and carers.
* Support the standardisation of discharge documentation, Policy and Procedure across Greater Glasgow & Clyde and demonstrate effectiveness.

Duration, Location, and Working Pattern:

* Permanent post
* Part time 14.5 hours
* Shift Pattern - Monday - Friday

Key Responsibilities

* Work autonomously, assessing, planning implementing and evaluating all aspects of discharge planning.
* Visit all wards on a regular basis to proactively support all members of the multidisciplinary team with simple and complex discharges.
* Education re: all aspects of discharge planning to all members of the multidisciplinary team. This is carried out both formally and informally.
* Co-ordination of the Discharge lounge where they exist.
* Day to day involvement with patients being discharged by the Scottish Ambulance Service.
* Liaise closely with all members of the multidisciplinary team, to ensure a cohesive, informed assessment re: patients being discharged home.
* Liaise closely with colleagues from supported discharge and bed management and other services to maximise the effective utilisation of beds.
* Maintain and improve good working relationships and communications with colleagues in primary and secondary care and in all local authorities.
* To be accountable for own professional actions as only indirectly supervised as a specialist in discharge.
* Assist with updating the Discharge Service Sharepoint.
* Take referrals from staff, patients and carers who require specialist advice.
* Participate in monthly orientation programme for qualified nursing staff.
* Provide information and advice on discharge policy and procedures.
* Ensuring excellent communication and sharing of information, with the relevant personnel to make decisions regarding a discharge plan.
* Actively involved in organising and implementing aids, equipment and post discharge support for the vulnerable patient, to ensure a safe and timeous discharge home.
* Actively involved in diffusing potential problems in relation to complex discharges i.e.; (verbal, abusive, threatening behaviour of patients, relatives and carers) by participating in both case conferences and at ward level.
* Formal collection and verification of data monthly for the Scottish Information Statistics Division re reasons for delay in discharges.
* Participate in audit of discharge and review national best practice in relation to discharge from hospital.
* Participates in data collection for audit purposes.
* Ensures that Continuous Professional Development meets statutory requirements and Knowledge and Skills Framework.

Knowledge, training, qualifications and/or experience required to do the job:

* Registered Adult Nurse (1st level)
* Minimum of five years post registration, with 2 years at Band 6 or equivalent within a ward/community environment.
* Proven managerial and clinical skills.
* Excellent working knowledge of discharge planning in Acute Services
* Excellent working knowledge of Acute Services in the Secondary Care setting
* Good working knowledge of Primary Care Services
* Evidence of continuing professional development
* Degree desirable
* Car driver essential
* Ability to fulfil criteria as outlined in job description.
