The role is based in the Mid Cornwall Integrated Care Board (ICB) with a focus on delivering high quality and effective mental health services to people experiencing severe and enduring mental health problems. This position requires travel to various locations across the county, including areas not easily accessible by public transport.
Eligibility: This job is currently only open to people who already work for the NHS in Cornwall or the Isles of Scilly and specifically to staff from the following organisations: Cornwall Foundation Trust; Royal Cornwall Hospitals NHS Trust; Cornwall and Isles of Scilly Integrated Care Board. Responsibilities
* Be responsible for the assessment of care needs and clinical management of a defined caseload within the Cornwall Partnership NHS Foundation Trust (CFT) Mental Health Service.
* Develop, implement and evaluate programs of care, utilising the care co-ordination system.
* Develop the interface between primary and secondary care services so patients/carers are offered focused and coordinated packages of care appropriate to their needs.
* Provide regular support and supervision within the Management and Leadership team framework.
Qualifications and requirements
* Core profession in Mental Health Nursing; experience working independently in a mental health setting is preferred.
* Knowledge of the CPA process, legislation around mental health, and the Mental Health Act; understanding of general mental health problems and their impact on service users.
* Ability to work flexibly as part of a team and demonstrate passion and commitment to mental health issues.
* Positive outlook and sense of humour to manage the challenges of working in a busy locality that includes seaside towns and rural locations.
Travel and location
This is a community-based role that requires frequent travel to various locations throughout the county. Successful candidates will need to demonstrate how they will manage these travel requirements.
