We are looking to recruit a highly motivated, innovative and enthusiastic Mental Health Support Worker - Young Adults Team to join our forward thinking, friendly and expanding Health & Justice team at HMP Bronzefield.
By joining our Prisons Health Care Team, you will look after the healthcare needs of patients residing in secure environments and have the opportunity to promote health and well-being. Our healthcare services operate on a 7-day week model to ensure healthcare is always managed to high standards. Our motto is “Caring NOT Judging”; we work in partnership with the criminal justice system and other agencies to provide responsive, dependable and high-quality primary care and mental health services to those who may have offended and those at risk of offending, in the communities we serve, to improve health and social inclusion.
The purpose of this post is to work as a member of the multi-disciplinary clinical team within a prison healthcare setting. Your role involves working compassionately with young adults with neurodevelopmental and mental health needs within the prison, either within the inpatient unit or on the main wings/houseblocks. You will focus on the holistic needs of service users, working across boundaries of care, organisations and roles coordinated through the Care Programme Approach within a trauma informed care framework.

Detailed job description and main responsibilities
Clinical:
* To work with the service user and carers in drawing up care plans, encouraging and motivating service users to take an active role in their own care plan.
* To contribute to the assessment, planning, implementation and review of care with the multi-disciplinary team
* Help identify early signs of relapse by monitoring progress, functioning and mental state and alert appropriate staff
* Assist in the delivery of the therapy programme within the service by co-facilitating groups
* Liaise with and refer service users to other healthcare services to ensure continuation of appropriate treatment
* Be actively involved in regular multi-disciplinary risk assessments along with daily informal risk assessments
* Report regularly to qualified staff
* Maintain accurate, contemporaneous patient records
* Participate in the review of the service provided and advise changes to the supervisor
* Undertake other duties within the general scope of the post
* Respect service users personal beliefs, individuality and identity

Education and Qualifications
* Care certificate or relevant equivalent
* NVQ Level 4 or equivalent in health or social care
