We are seeking an experienced Adults Social Worker to join the Locality Team in Cheltenham for an ongoing opportunity. What you will do
* Manage a varied caseload of adults with a range of care and support needs within the community.
* Undertake assessments, care planning, and reviews under the Care Act 2014.
* Work collaboratively with health professionals, families, carers, and other agencies to develop person-centred care plans.
* Apply a strengths-based approach to practice, promoting independence and empowerment.
* Ensure safeguarding procedures are followed and contribute to safeguarding enquiries when necessary.
* Maintain accurate and timely case notes and records in accordance with local and national guidelines.
* Participate in multidisciplinary team meetings and support service users through care transitions.
Requirements
* Qualified Social Worker with registration with Social Work England.
* Sound knowledge of legislation relevant to adult social care, including the Care Act 2014, Mental Capacity Act, and Safeguarding Adults.
* Experience of working within a Locality or similar adults’ team in a local authority setting.
* Strong communication, assessment, and interpersonal skills.
* Ability to work independently and manage workload efficiently.
* Enhanced DBS check required.
