Huntington's Disease Specialist Nurse – Livewell Southwest (Band 6) – Fixed‑term 12‑month, full‑time position for a registered nurse with expertise in Huntington’s disease and a passion for delivering high‑quality, person‑centred care.
Job Summary
37.5 hours per week. Fixed‑term 12‑month, full‑time Band 6 role for a registered nurse with expertise in Huntington’s disease. You will support patients and families in the community, collaborating with the Older Peoples Mental Health (OPMH) Functional Team to deliver holistic, person‑centred care.
Main Duties
* Provide expert nursing assessment, care planning and ongoing support for individuals living with Huntington’s disease in the community.
* Act as the primary point of contact for patients and carers, offering advice, education and emotional support.
* Coordinate services with the OPMH Functional Team and other professionals to ensure integrated, holistic care.
* Plan, implement and evaluate care delivery, including risk assessment and management.
* Contribute to multidisciplinary clinics, hospital and care‑home liaison, and facilitate early referral to services.
* Support research, audit and service development initiatives within the Huntington’s disease service.
* Provide education and training to patients, carers and health‑care professionals, and promote awareness of current clinical trials.
Qualifications – Essential
* Registered Nurse with current NMC registration.
* Post‑graduate specialist knowledge and/or experience in a community mental health setting.
* Evidence of recent professional development.
Qualifications – Desirable
* Ongoing mandatory training.
* Knowledge of HD: An enabling approach to supporting families.
* Continued professional development.
Key Skills & Experience
* Skilled clinician with experience in assessment, risk management and crisis intervention.
* Excellent verbal and written communication, empathy, interpersonal, supervisory and leadership skills.
* Ability to work autonomously within an MDT, manage stress and change, and support service development.
* Experience working with community health or hospital settings, and with adults diagnosed with Huntington’s disease.
* Knowledge of NICE guidelines, UK legislation, and Livewell policies relevant to the role.
Contract & Working Pattern
Fixed term, 12 months. Full‑time (38.5 hrs). Flexibility across a 7‑day service. Salary: £38,682 – £46,580 per annum, pro‑rata.
