Waythrough Community Diversion Service - Hospital Link Worker
Waythrough launched in October 2024 following the merger of Humankind and Richmond Fellowship. Our vision is to break down barriers that prevent people from accessing support to live a valued life. We address poverty and disadvantage through mental health, substance misuse, housing, and related support services.
We operate nearly 200 services across England with around 3,500 staff and volunteers, supporting approximately 125,000 people annually.

About the Role
The Community Diversion Service, funded by the Northumbria Violence Reduction Unit, aims to ensure Northumbria is a safe and thriving place where individuals, families, and communities can flourish. The service supports children and young people aged 10 to 25, along with their families, who are vulnerable or at risk of serious violent crime.
Role Summary
We are seeking two dedicated Hospital Link Workers to support individuals presenting at Northumberland Hospital A&E with injuries related to serious violent crime. The role involves motivating service users to make positive changes and engage with specialist services to achieve better outcomes.
Key Responsibilities
* Caseload Management: Support and manage a caseload of service users, providing tailored assistance.
* Intervention Planning: Develop and review support plans collaboratively with service users.
* Collaborative Approach: Work with a multidisciplinary team, families, and significant others.
* Goal Setting: Help service users set and work towards achievable goals.
Note: Two vacancies are available in Northumberland. Post holders must have access to a car and be able to commute to a Newcastle-based office as required.
This role offers a hybrid working pattern, with shifts including community work and hospital shifts (Friday and Saturday evenings). Flexibility may be required to meet service users' needs.
About You
* A commitment to the well-being and rehabilitation of service users.
* Experience in substance misuse, support work, mental health, or criminal justice sectors.
* Excellent communication and interpersonal skills.
* Ability to work effectively within a multidisciplinary team.
* Knowledge of safeguarding practices for children and vulnerable adults.
* Adherence to ethical standards and guidelines.
