Overview

We have a very exciting opportunity to join our integrated Heart Failure Service at Calderdale and Huddersfield NHS Trust. We are seeking a highly motivated individual with excellent interpersonal and communication skills. The successful candidate will be responsible for the development of a cohesive management programme for patients with heart failure in line with local and national guidelines, working across acute, community and Hospital at Home settings. The service will operate 7 days a week, with a view to extending hours of 8am to 8pm in the future and requires flexible working within an evolving proactive, dynamic, and committed multidisciplinary Heart Failure Team.

The aim of the role is to work autonomously, within the team, managing own caseload and to identify patients who can be supported on the Heart Failure Virtual Ward, reducing length of stay, preventing unnecessary admissions to hospital, and improving heart failure management and control with optimisation of evidence-based treatments.


Responsibilities

* Work autonomously within the Heart Failure Team to manage own caseload.
* Identify patients suitable for the Heart Failure Virtual Ward and support ongoing care for patients actively managed on the Hospital at Home pathway.
* Develop the specialist area of practice in partnership with the lead clinician, matron, service lead, and other members of the specialty team.
* Act as the patient advocate at all times and contribute to the development, implementation, and evaluation of programmes of care.
* Set standards of care, assess care needs, and work within the clinical governance framework.
* Support the Hospital at Home service and use specialist training, knowledge, and expertise to diagnose, assess, and deliver evidence-based care, including self-management education for patients.
* Identify opportunities to extend services across inpatient, outpatient, community, and Hospital at Home settings.


Qualifications and Requirements

* Experienced community practitioner with diagnostic and consultation skills.
* Preferably a non-medical prescriber or willing to complete the course.
* Ability to work across acute, community, and Hospital at Home settings with flexible working patterns.

Additional Notes

Will work across the hospital with responsibility for patients with heart failure in the inpatient, outpatient, community, and Hospital at Home settings. Will be responsible for the early identification of patients for the Heart Hospital at Home ward from the acute setting and supporting ongoing care for patients who are actively managed on the Hospital at Home.

The post holder will take overall responsibility to develop the specialist area of practice in partnership with other members of the specialty team and will work collaboratively to develop services and quality of care delivered. The post holder will be expected to act as the patient’s advocate at all times and to carry responsibility for their defined caseloads and provide highly specialist service advice, including setting standards of care, assessing care needs, and developing, implementing, and evaluating care programmes. They will contribute to achieving the objectives of the role within the clinical governance framework and will work autonomously to support the Hospital at Home.
