The NursePractitioner in primary care offers direct access to clients seeking healthcare. To be able to undertake initialassessments of any health problems likely to be encountered in a primary healthcare setting and, following assessment, can initiate treatments falling withinher range of knowledge and skills. TheNurse Practitioner can admit people to the primary health care system, both byoffering initial treatment herself and by referral to others, includingphysicians.

The NursePractitioner evaluates the effectiveness of treatment she has carried out atsuitable intervals and, when appropriate, will discharge her clients from theprimary health care system.

As apractitioner of nursing, she will offer an educational and supporting settingfor clients to explore their health problems and will work with them for aslong as necessary, enabling them to regain health.


Main duties of the job

* Make professional autonomous decisions. Will have independent case loads.
* Work closely with other members of theprimary health care services.
* Exercise the right to refer patients toappropriate services and resources
* Maintain clinical notes in the patientsmedical records according to legal requirements.
* Demonstrate knowledge and skill ininterviewing and history taking and record appropriate details
* Perform physical examinations
* Select, carry out and interpret results ofspecific tests.
* Develop with the patients an on-going planfor health,
* Provide counselling and health education.
* Co-ordinate the care provided for herpatients by other carers.
* Evaluate the effectiveness of the careprovided in terms of expected outcomes.
* Collaborate with other members of theprimary health care team
* Assist in the planning, development,implementation and evaluation of preventative programmes

We are looking for aproactive, highly motivated individual who is fully competent in all aspects ofpatient care and dedicated to delivering a first class service in a primarycare setting.

Job responsibilities

* Make professional autonomousdecisions, for which she has sole responsibility.
* Will have independent case loads,seeing patients with undifferentiated and undiagnosed problems, diagnoseand, if necessary, seek advice and guidance from one of the medicalpractitioners whilst providing care herself.
* Work closely with other members ofthe primary health care services, respecting professional role boundariesand acting in the best interests of patients, and liaise independentlywith agencies outside the practice.
* Exercise the right to referpatients to appropriate services and resources within and outside thepractice. These include other professionalsby referral and an agreed range of services.
* Maintain clinical notes in the patientsmedical records according to legal requirements.
* Demonstrate knowledge and skill ininterviewing and history taking and record appropriate details of her findingsin the patients medical records.
* Perform physical examinations,where necessary, and assessments, where appropriate, to recognizeabnormalities and screen patients for risk factors and early signs ofillness.
* With in-house training, implementand evaluate treatment/intervention plans for each patient, based upon acomprehensive nursing assessment and seek and refer patients for treatmentto medical practitioners in the practice, where necessary. For specific diseases, where necessary,organize recognized training, e.g. diabetes, asthma, epilepsy,hypertension etc.
* Select, carry out and interpretresults of specific tests.
* Develop with the patients anon-going plan for health, with emphasis on preventative measures.
* Provide counselling and healtheducation as appropriate to facilitate health promotion.
* With the patients` consent,discuss with other health care professionals and, if appropriate,relatives, the assessment/goals and treatment for patients.
* Co-ordinate the care provided forher patients by other carers.
* Evaluate the effectiveness of thecare provided in terms of expected outcomes.
* Collaborate with other members ofthe primary health care team in establishing protocols for the managementand care of patients within all nursing and nurse practitioner`s fields ofpractice.
* Assist in the planning,development, implementation and evaluation of preventative programmes tomeet the primary health care needs of the practice.
* Act as a nurse consultant to othermembers of the health care team, providing clinical advice and support toother professionals within the health care setting and to externalagencies regarding social issues etc.
* Participate in practicemanagement, e.g. management of practice nurses, health needs assessmentetc.

Qualifications

* Nurse Practitioner qualification working at degree level
* working within General Practice/GP Surgery


Experience

* Nurse Practitioner qualification working at degree level
