Hartlepool Borough Council are seeking torecruit an enthusiastic and motivated health visitor, applicants must hold orbe working towards a specialist community public health nurse (SCPHN)qualification in Health Visiting.


Main duties of the job

As a SCPHN health visitor you will lead and develop a family centered public health approach, working with individuals, families and communities to improve health and tackle inequalities.

You will be committed to deliver evidencebased practice, maintaining current knowledge of local and national issues thatimpact on the 0-5 population. As a specialist practitioner you will be requiredto professionally contribute to the on-going development of the health visitingoffer within the Early Help Locality Teams.

You would be supported to fulfill your rolethrough induction, training, shadowing and a preceptorship package tailored to yourindividual needs.

Job responsibilities

To work as part of anintegrated locality 0-19 Early Help team providing an effective high qualityservice to the local 0 - 5 population and their families.

To effectively managean allocated caseload with support from the integrated locality team, whichwill promote the health and wellbeing of the 0 - 5 population, so they may beenabled to reach their full potential and not be disadvantaged through illhealth or disability.

To use evidence-basedpractice to plan, deliver and evaluate public health interventions in a varietyof settings.

Person Specification


Knowledge and Skills

* Proven experience to organise and manage workload
* Able to demonstrate developed written and verbal communication skills
* Flexibility able to cope with different work environments
* General computer skills, including use of word, excel and powerpoint
* Knowledge and previous use of SystmOne electronic patient record
* Car Owner and Driver
* A positive attitude towards integrated working
* Evidence of team working


Experience

* Evidence of caseload management, managing time and resources
* Evidence of continuing professional development
* Ability to demonstrate effective communication and interpersonal skills
* Be able to challenge traditional boundaries using evidence-based approach
* Ability to demonstrate change management skills
* Evidence of awareness of health needs assessment and health inequalities
* Evidence of knowledge of policies impacting on the health of children and families
* Ability to demonstrate influencing and negotiating skills
* Experience of safeguarding children and supporting vulnerable families
* Evidence of facilitation students undertaking the SCPHN programme with in health visiting


Qualifications

* Specialist Community Public Health Nurse - Health Visiting or currently studying to become a SCPHN HV
* Registered Nurse or Midwife
* Nurse prescribing
