Job overview

Opportunity to join the District Nursing Service as a Community Staff Nurse in the Tweedmouth District, based in Berwick. The role provides patient‑centred care in patients’ homes, residential care settings and clinics, working closely with GP surgeries.

Key responsibilities

* Work autonomously and as part of a team, reporting directly to the line manager or community nursing team lead.
* Assess, plan, implement and evaluate care for a diverse group of patients with a wide range of conditions.
* Develop and deliver health and social care programmes.
* Provide care and support to patients with terminal illness and their families, including emotional and bereavement support.
* Deliver skilled nursing care: assessment, wound management, palliative care, catheterisation and other clinical procedures.
* Perform diagnostic tests such as Ankle Brachial Pressure Index using Doppler; interpret results to inform further action.
* Perform fine‑dexterity procedures – venepuncture, drug admin by injection, removal of sutures.
* Interpret and communicate laboratory and clinical results to patients and carers, explaining care aspects and addressing concerns.
* Maintain high standards of professional practice and share best practice with colleagues.
* Exercise duty of care and uphold personal and professional accountability.


Qualifications

* State registration on Part 1 of the NMC register.
* Additional clinical qualifications or equivalent (diploma/degree level) if experienced.
* Newly qualified staff must undertake the Trust preceptorship programme within the first year.
* Evidence of qualification to supervise, teach and assess students (required within 18 months).


Experience and knowledge

* Experience working without direct supervision in a healthcare environment.
* Commitment to Compassion in Practice – Nursing, Midwifery and Care Staff Vision and Strategy, incorporating the 6C’s.
* Knowledge or experience in coaching and mentoring practices and tools.
* Knowledge or experience in quality improvement tools, techniques and methods.


Other requirements

* Valid driving licence and ability to use a car for work purposes or a Trust personal lease vehicle; reasonable adjustments may be considered.
