Overview
This is a Band 6 Community Learning Disability Nurse position within the Wrexham area. The post holder will join the Wrexham Learning Disability Community Health team at Betsi Cadwaladr University Health Board and will work to ensure interfaces between the Local Authority and neighbouring services within the Division, including community mental health teams, in-patient services, other statutory agencies and the voluntary sector.
The post holder will deliver evidence-based, safe, effective interventions and clinical care, following the nursing process. The service principles and strategy for people with a learning disability will be followed while delivering a safe, high-quality specialist service in line with agreed standards.
Responsibilities
The post holder will:
* Be a visible, accessible member of the team and provide patients and their families with assistance, advice and support.
* Communicate and receive highly complex and sensitive information; analyse and interpret information and act appropriately where there may be barriers to acceptance.
* Have a good understanding of the CHC process and contribute to the development of pathways and the Maturity Matrix.
* Demonstrate excellent communication skills and deliver services in Welsh and/or English as required.
Location and Inclusions
Location: Wrexham Crown Building, 31 Chester Road, Wrexham, LL13 8BG. Salary: £39,263 to £47,280 per annum. Band 6. Contract: Permanent. Working pattern: Full-time. Date posted: 05 September 2025.

Qualifications and Experience
Essential:
* RNLD Qualification.
* Evidence of a degree-level qualification or equivalent in line with NMC registration, with evidence of postgraduate education and ongoing CPD.
* Significant clinical nursing experience in the relevant specialty or a related nursing role.
Desirable:
* Management qualification and/or management experience.
* Mentor/supervisor/assessor experience.
* Experience of working in a multidisciplinary team and knowledge of senior nurse roles.
Aptitude and Abilities: Essential
* Ability to communicate and receive complex information.
* The ability to diagnose problems and take corrective action; analyse assessment results and translate them into appropriate packages of care.
* Ability to work autonomously as well as part of a team.
Desirable
* Ability to speak Welsh; IT skills; ability to manage resources.
