Overview
The Pennine Care Community Rehabilitation Team is seeking a motivated and enthusiastic Team Manager to lead a multidisciplinary team. The successful candidate will have significant experience of working with people with chronic relapsing and remitting mental ill health, including experience across secondary care services.
The team supports service users to transition from inpatient rehabilitation facilities to living in the community, aiming to improve quality of life and help service users work towards their goals. A key aim is to reduce inpatient stay lengths on acute and rehabilitation wards, incorporating least restrictive practice and NICE guidelines for complex psychosis. The team emphasises the Recovery Model, using strengths-based and trauma-informed approaches to maximise independence and autonomy.
You will receive regular supervision and development priorities will be supported through the appraisal process. The team is a trust-wide service covering all five boroughs of Pennine Care; travel across the trust footprint is essential. We are based at Trust HQ in Ashton Under Lyne, with potential changes. Core hours are Monday to Friday, 9am–5pm.
