Role Overview
Assistant Practitioner in a Multidisciplinary Community Therapy Service at Bolton Foundation Trust, working across six neighbourhoods. The service operates 7 days, 8am‑6pm; Band 4 assistants do not currently take part in weekend service.
Key Responsibilities
* Independently assess and treat patients, managing your own caseload.
* Carry out specific care and treatment interventions and adjust or develop them as indicated, with supervision from qualified staff as required.
* Supervise junior staff where appropriate.
* Collaborate as part of a multi‑disciplinary team consisting of Physiotherapists, Occupational Therapists, Therapy Instructors and Assistant Practitioners.
* Support the NHS 10‑year plan by facilitating hospital‑to‑community transition, incorporating digital solutions and shifting focus from treatment to prevention.
* Provide assessment, personalised care planning and management for patients needing intensive home support as an alternative to hospital care.
* Progress and discharge patients from the service under direct supervision of a registered therapist as required.
Qualifications & Skills
* Band 4 status within the NHS pay banding system.
* Excellent communication and organisational skills.
* Dynamic, confident, innovative, flexible, and able to work independently while maintaining teamwork.
* Ability to manage a caseload and work autonomously across a variety of settings.
