As a Community Support Worker, you'll play a vital role in our service. You will work closely with the multi‑disciplinary team made up of clinicians such as physiotherapists and nurses to provide ongoing clinical activities such as nursing tasks, rehabilitation, recovery and support with daily routines and personal care. Successful candidates will have the support of their teams to be comfortable working independently in the community.

Responsibilities
* To assist registered nurses and healthcare professionals in the delivery of individualised holistic care which is delivered with compassion, dignity and respect.
* Work with the multidisciplinary team and contribute to the prevention of harm by participating in risk assessments such as nutrition, pressure area care, continence and falls.
* To assist patients with their care and nutritional support and needs.
* To safely and accurately record physiological observations such as blood pressure, pulse, respirations and temperatures and to escalate and report to the registered nurse accountable for the patients’ care.
* To report back any relevant care information about the patient to the registered nurse.
* Assist patients to mobilise safely using equipment and techniques prescribed and recorded in their plan of care.
* Promote a culture of ambulatory care by supporting patients to be as independent as possible with washing, dressing in own clothes and encouraging mobility from admission.
* Assist in the provision of prescribed programmes of care such as rehabilitation and therapy goals under supervision and guidance.
* Support patient‑centred rehabilitation to facilitate safe and timely discharges and to reflect individual choice and maximize independence.
