Eastbourne East Primary Care Network has an excitingopportunity for an experienced paramedic to join our expanding team of GPs,Practice Nurses, Advanced Practitioners, Clinical Pharmacists, First ContactPractitioners and Care coordinators.As a member of our ARRS team, you will play a vital role in the delivery ofcare to our patients, particularly those who are house bound or in Care Homes.You will also contribute to the care of patients on site, in the surgeries of ourthree network practices by running your own surgeries and participating in othersurgery activities like clinical audits, QOF related work, enhanced accessservices etc, when required.


Main duties of the job

Thepost holder will be an experienced clinician, who, acting within theirprofessional boundaries, will provide clinical assessment and management ofpatients in their own home, supported by GPs as required. The post holder mayalso be required to see, assess and manage patients with urgent healthconditions within a GP practice clinic setting as part of their developmentprogramme.The post holder will provide and maintain a high standard of care for patientsas well as providing support to the GPs and other members of the primary healthcare team. They will be supported by other members of the Primary Care Team.The main role will be to work within the community, as an autonomous,accountable, Paramedic Practitioner.


About us

Eastbourne East PCN is a network of three GP Practices Sovereign Practice, Stone Cross surgery and Harbour Medical Practice. We work closely collaboratively to deliver care to our population of approximately 37,000 residents, supported by e3 Healthcare Ltd, our own exclusive Limited Liability Company.

We have awell-established clinical team who support and provide excellent care to ourPCN patient population. We currently have a good blend of clinicians but wantto build on our PCN team. Peer support will be available from the alreadywell-established practice teams of paramedics and advance nurse practitioners.


Job responsibilities

* To visit and manage house-bound patients as requested by GPs, in order to holistically assess the patient, diagnose where possible, signpost, treat or refer the patient to other appropriate services.
* If the post holder has completed an independent prescribing qualification, to safely prescribe appropriate medication for patients from a pre- approved list, when necessary.
* To manage patients in GP surgery premises by face-to-face consultation and assessment or when safe and appropriate via remote consultation and assessment.
* Participate in the PCNs Enhanced Access services by running surgery-based clinics for patient assessment and management, when required.
* Take a Lead role in medical emergency situations whenever the post holder is available on site a the time of the incident
* Contribute to and participate in Quality Improvement Activities of the PCN Network practices eg Clinical audit work, QOF related work, PCN IIF work, etc
* Participate in all Safeguarding matters as may be required of the post holder by the PCN or GP practices
* To identify the limitations of their own practice and liaise with senior colleagues where these limitations have been reached to ensure good quality patient care is maintained.
* To use advanced clinical skills to provide education to patients, promoting self-care and empower them to make informed choices about their treatment.
* Maintain accurate clinical records ensuring that Read Codes are used effectively and accurately.
* Participate in the PCN and Practices educational and developmental activities
* Ensure that professional standards are maintained and within the guidance by the Department of Health, the Health and Care Professions Council (HCPC) and the College of Paramedics (COP)
* Work as an effective and responsible team member, supporting others and exploring the mechanisms to develop new ways of working.
* Where involved in delegation of tasks, delegate clearly and appropriately, adopting the principles of safe practice and assessment of competence of those taking on delegated duties.
* Ensure clear understanding and utilisation of referral mechanisms within the practice.
* Prioritise own workload and ensure effective time-management strategies are embedded in own practice.
* Working with the wider Multidisciplinary team to provide a holistic approach to care.
* Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating.
* Be confident undertaking lone working sessions.
* Undertake mandatory and statutory training.
* Apply policies that reduce environmental health risks, are culturally sensitive and increase access to health care for all.
* Recognise and work within own competence and professional code of conduct and other requirements as regulated by the Health and Care Professions Council (HCPC) and the College of Paramedics (COP) and to ensure familiarity with and adherence to those requirements.
* Maintain own CPD and contribute to own personal development by participating in annual appraisal with line manager, developing a PDP, and actively participating in agreed learning activities and evaluating effectiveness of learning in relation to role.
* Regularly review own practices and make changes in accordance with current and/or best practice, makes suggestions for improved practice and identifies where other activities affect own practice.
* To achieve and demonstrate agreed standards of personal and professional development within agreed timescales.
* To act in ways that support equality and diversity and the rights of individuals, ensuring own practice is in accordance with organisational policies. Identify and take action when one's own or others action undermines equality and diversity.
* To raise concerns around risk, malpractice or wrongdoing at work, which may affect patients, staff or the organisation, at the earliest reasonable opportunity.
* To maintain the highest standards of care and service, treating every individual with compassion, dignity and respect, taking responsibility not only for the care you personally provide, but also for your wider contribution to the aims of your team.
* The post holder will be required to adapt to and undertake different or new duties in line with professional and service developments.
* To supervise and support junior members
* Report and record all clinical incidents.


Person Specification


Qualifications

* Experience of working in Primary Care and/or a GP Practice.
* Experience of selecting and prescribing a wide range of medicines appropriately, including antibiotics and analgesia.
* Provide basic and advanced life support.
* Providing general clinical advice and wider health promotion.
* Ensure safeguarding of vulnerable patients.
* Provide clinical support to all colleagues and learners in practice.
* Full registration with the Health Care Professions Council (HCPC).
* Higher Education Qualification (level 7).
* Familiarity with EMIS Clinical Record System
* Ability to create and maintain strong professional relationships


Experience

* Paramedic Practitioner - minimum 1 year
* Higher Education Qualification (level 7)/degree or equivalent experience required.
* HCPC registration.
* Be verified against the paramedic primary care roadmap.
