Care Act Assessor
Midlands Partnership NHS Foundation Trust has an opening for a Care Act Assessor role within the East Staffs IRT Adult Social Care Team. Applicants will be based at Edwin House, Burton, with blended working between home and office.
The role involves telephone screening of referrals, completing statutory Care Act assessments under the supervision of the Practice Lead, and working closely with service users, carers and other professionals to determine the level of response required. It is a fast‑paced, demanding role requiring proactive communication and organisational skills with knowledge of the Care Act 2014.
Key Responsibilities
* Telephone screening of referrals received by IRT.
* Complete Statutory Care Act assessments under supervision.
* Collaborate with service‑users, carers, partner agencies and professionals to assess and respond to referrals.
* Participate in continuous professional development and maintain SWLA alignment.
* Drive and use business vehicle as required.
Qualifications & Training
* Level 3 Diploma in Adult Care (RQF) or equivalent, with additional knowledge to diploma level.
* Level 4 Diploma in Adult Care (RQF) or equivalent.
* Good computer literacy and IT skills.
* Commitment to personal and professional development.

Experience
* Demonstrated commitment to partnership working with adults, carers, professionals and voluntary agencies.
* Experience working in a team, maintaining good interpersonal relationships and contributing to service improvement.
* Experience within the specialist area of adult social care.
Other Requirements
* Driving licence and vehicle for business use.
* Subject to Disclosure and Barring Service check and Rehabilitation of Offenders Act (Exclusions Order) 1975.
Works Hours and Salary
£27,485 to £30,162 per annum (pro‑rata if part‑time). Commences: includes potential blended working.
