The Looked after children's health team promotes and monitors the health and well-being of looked after children and young people.

The roles includes working collaboratively across the Health and Social Care setting with a wide range of professionals across health, social care, education and voluntary sector to achieve the best health outcomes for children and young people looked after. This is to ensure that looked after children and young people receive the appropriate services in order to reach their full potential, as outlined in Promoting the Health and Wellbeing of Looked after Children (2015)

The role also requires the need to develop, co-ordinate and facilitate access to a broad range of health services in order to ensure looked after children, care leavers and unaccompanied asylum-seeking children's needs are met and positive outcomes for this vulnerable group are achieved.

The role also requires linking with the child or young persons named social worker or personal assistant and other professionals and will work actively as a member of a multi disciplinary team, whose priority is the care of looked after children and young people.

The service is led by the Named Doctor and Nurse for Looked After Children. The team is responsible for co-ordinating and arranging statutory health assessments for children and young people placed in and out of borough to ensure they have the best possible outcomes.

We are seeking to recruit a dynamic and innovative nurse to join the Looked after Children's Team. The role involves being responsible for co-ordinating health needs review assessments; ensuring individual packages of care are formulated and implemented.

The role requires you to provide support and training to a wide range of professionals in order to promote the health of looked after children, care leavers and unaccompanied asylum-seeking children.

The expectation is that the nurse will ensure that all local and national policy initiatives are reflected in the service delivered.

We strive to recruit and retain staff who are motivated to develop and learn with us as we are continually strive to ensure that our services are of the highest quality, safe and focused on children and families. The role requires that you are aware of their responsibilities by attending the Trust Mandatory Training and Induction Programme.

The post holder will be involved in a formal appraisal and probationary review with the manager at least every 6 to12 months. Once performance / training objectives have been set, the staff member's progress will be reviewed on a regular basis, so that new objectives can be agreed and set, in order to maintain progress in the service delivery.

The nurse will be expected to take responsibility for their own professional development and will be supported by the Trust to achieve development opportunities as appropriate
