Job Overview
This is an exciting role for an experienced Registered Nurse to work within the multidisciplinary frailty team at Frimley Park Hospital. The Frailty Service provides a front‑door assessment and inpatient service, offering comprehensive geriatric assessments, complex care plans and advice to older people attending the acute hospital services with a frailty syndrome. The team aims to support admission avoidance, reduce length of stay, and avoid unnecessary re‑admissions for individuals living with frailty. You will work collaboratively with, and be supported by, the Consultant Nurse, Consultant Geriatricians, GPs and a team of multidisciplinary frailty practitioners that includes nurses, medics, physiotherapists, and a pharmacist.
As a registered nurse, you will spend time in patient‑facing roles in the Emergency Department, the Frailty SDEC, Acute Medical Unit, as well as inpatient wards, to identify patients with frailty early in their journey to provide holistic assessment in the form of a Comprehensive Geriatric Assessment and prevent hospital admissions.
This post would suit a Registered Nurse with experience of elderly care, emergency medicine and/or a rehabilitation background, who is seeking an opportunity to enhance their understanding of frailty and the management of this patient cohort in the acute environment.
Training will be provided to ensure that you are able to complete all aspects of the frailty practitioner role under the guidance of a senior clinician.
Main Duties
* Work closely with the Consultant Nurse for Older People, Frailty Practitioners, in‑reach GPs and the Consultant Geriatrician team to provide fast reactive services for patients with decompensated frailty and ensure rapid delivery of treatment, care planning and hospital admission prevention where appropriate.
* Provide advanced assessment and care planning, including history taking and physical assessment for patients with frailty.
* Proactively identify and manage patients with frailty and support them and their carers in the development of supportive management plans.
* Provide evidence‑based holistic assessment and treatment planning for patients with frailty referred to the service, without direct supervision.
* Work in conjunction with a wide range of clinical colleagues – primary care and community teams and Social Care professionals – leading and facilitating a patient or client focused, coordinated case management approach across primary and secondary care for people who are most vulnerable to and at high risk of repeat admissions to hospital.
* The frailty service operates 7 days a week, currently 07:30‑19:30 on Monday to Friday and 07:30‑17:30 on Saturday and Sunday. The post holder will be required to work within these operating hours, including sharing weekend and bank holiday cover with the other members of the team.
Person Specification
Qualifications
* Registered General Nurse.
* Post‑qualification or equivalent experience gained by undertaking ongoing personal development and training.
* Teaching and assessing qualification/mentorship qualification e.g. PETALS.
Desirable Qualities
* Management/leadership qualification or developmental programme.

Experience
* Experience, underpinned by knowledge of working with and understanding the complex needs of patients in a primary care / community setting.
* Post‑registration experience in a range of clinical areas.
* Experience of caseload management including responsibility for complex care packages for vulnerable people.
* Experience of working with long‑term conditions and frailty.
Desirable Experience
* Involved in the implementation and management of change.
* Experience of initiating or participating in clinical audit/research relating to clinical practice.
