First Dietitians is a team of 24 dietitians, dietetic assistants and administrators delivering adult and paediatric services across East Surrey and parts of West Sussex in a variety of community settings. Our vision is to be a trailblazer for food and nutrition through public involvement, quality improvement and innovative new service models. You will support our Sussex and Surrey adult teams in clinics, domiciliary visits and ward support, and act as a key member of the multi‑disciplinary team to build relationships with patients and their families.
Responsibilities
* Contact patients by phone, e‑mail or letter to book education sessions, clinic appointments and home visits.
* Visit patients in care homes or their own homes to conduct nutritional assessments, including weight and height checks.
* Work with lead community dietitians to improve the service.
* Support dietitians with everyday tasks.
* Conduct ward reviews and gather information with support from the dietetic team.
* Send out nutritional samples for clinic patients.
* Conduct weekly nutritional support clinics with the support of the dietetic team.
* Order ancillaries for enteral feeding patients and communicate with the local feeding company on behalf of patients.
