The service typically manages patients recovering from: Acute illness Exacerbations of long-term conditions Reduced mobility or functional declineThe unit provides essential step?down care for individuals discharged from acute hospitals but not yet ready to return home. The focus is on: Medical optimisation Functional recovery Effective discharge planning Reducing the risk of hospital re-admissionAlthough the setting is technically secondary care, the model is GP/ANP-led, not consultant-led, and care is delivered by a full multidisciplinary team, including: GPs / ANPs Nursing staff Physiotherapists & Occupational Therapists Pharmacy teams Discharge coordinatorsKey Responsibilities As the ANP, you will play a crucial clinical role in managing patients within the unit.
