This role plays a vital part in delivering strength-based, person-centred social care, promoting wellbeing, independence, and positive outcomes for adults with care and support needs. About the Role: As a Social Care Assessor, you will: Manage a varied caseload using strength-based approaches Carry out social care assessments, support planning, and reviews Work collaboratively with individuals, carers, health professionals and other stakeholders Ensure care planning reflects individual outcomes and is aligned with statutory duties Contribute to safeguarding, risk assessments, and mental capacity evaluationsWhat We’re Looking For: Experience in adult social care or related settings Ability to deliver assessments and support planning with empathy and clarity Strong organisational, communication and partnership-working skills Understanding of adult care legislation and safeguarding practices IT competence and a commitment to equality, diversity, and customer careDesirable (but not essential): NVQ Level 3 or 4 in Health & Social Care or Promoting Independence Experience with multicultural communities and budget monitoringWhy Join Us?
