Overview

The Stoma/ERAS Clinical Nurse Specialist (CNS) is a key worker role to address patients' health concerns and promote their health and wellbeing. Using expertise in Stoma Care, the CNS provides physical and emotional support to patients, coordinates their care services, and informs and advises them on clinical and practical issues associated with their illness. This role supports the implementation of the Enhanced Recovery pathway and the CNS role dimensions of expert clinical practice, education, leadership, management and research.


Responsibilities

* Assist in the provision of professional and clinical leadership and expertise in the care of the patient with a gastrointestinal stoma, acting as a resource to provide information and support to patients, carers and staff.
* Liaise effectively with all members of the Hospital Multidisciplinary Team and Community Team to achieve a seamless specialist service in stoma care.
* Help to achieve delivery of gold standards of care to reduce length of hospital stay and readmission rates to achieve parity with the NCL providers.
* Lead and contribute to the creation of patient information.
* Support implementation of Enhanced Recovery pathway of care for colorectal patients and work with the Multidisciplinary Team to ensure that all aspects of Enhanced Recovery pathway are optimised.
* Provide formal and informal training and education within pre-assessment and ward areas to embed the ERAS pathway of care.
* Lead and contribute to the development of clinical guidelines and policies both internally and externally.
* Provide telephone follow up of patients following discharge from enhanced pathways of care; act as a keyworker for a clinical caseload, coordinating care and providing ongoing support to patients.
* Provide professional and clinical leadership, ensuring delivery of excellence in core, specialist and advanced nursing practice, audit, research, and service and practice development agendas.
* Identify outcome measures to illustrate the quality of the service with emphasis on patient-related outcome measures.
* Recognise and champion innovations in nursing practice, ensuring they are supported, evaluated and contribute to developing an evidence base to meet patient, family, and staff needs.
* Communicate highly complex and sensitive information to patients, carers and other staff.
