Main duties of the job
- Conducting comprehensive assessments of patients, including physical examinations, medical history reviews, and diagnostic tests
- Formulating and implementing treatment plans, including prescribing medications, interpreting test results, and providing ongoing patient education and support
- Managing and coordinating care for patients with complex or chronic conditions, working closely with physicians, specialists, and other healthcare providers
- Participating in quality improvement initiatives, research projects, and professional development activities to continuously enhance clinical practice and patient outcomes

Job responsibilities
Thefollowing are the core responsibilities of the Advanced Nurse Practitioner.There may be, on occasion, a requirement to carry out other tasks; this will bedependent upon factors such as workload and staffing levels:
a. Develop, implement and embed healthpromotion and wellbeing programmes
b. Manage patients presenting with a rangeof acute and chronic medical conditions, providing subject matter expert advice
c. Implement and evaluate individualspecialised treatment plans for chronic disease patients
d. Identify, manage and support patients atrisk of developing long-term conditions, preventing adverse effects to thepatients health
e. Provide advanced, specialist nursingcare to patients as required in accordance with clinical based evidence, NICEand the NSF
f. Provide wound care (ulcer/Doppler, etc.)
g. Provide specialist clinics such as earsyringing, contraception, etc.
h. Undertake the collection of pathologicalspecimens
i. Provide travel medicine services
j. Request pathology services as necessary
k. Process and interpret pathology andother test results as required
l. Provide chronic disease clinics,delivering patient care as necessary, referring patients tosecondary/specialist care as required
m. Maintain accurate clinical records inconjunction with extant legislation
n. Ensure SNOMED CT codes are usedeffectively
o. Maintain chronic disease registers
p. Develop, implement and embed well womanclinics
q. Develop, implement and embed well manclinics
r. Develop, implement and embed travelclinics
s. Develop, implement and embed aneffective call/recall system
t. Be responsible for the implementation ofan effective immunisation programme
u. Chaperone patients where necessary
v. Prioritise health issues and interveneappropriately

y. Implement vaccination programmes foradults and children
z. Be an extended and supplementaryprescriber, adhering to extant guidance
aa. Support patients in the use of theirprescribed medicines or over the counter medicines (within own scope ofpractice), reviewing annually as required
bb. Contribute to practice targets (QOF,etc.), complying with local and regional guidance
cc. Liaise with external services/agenciesto ensure the patient is supported appropriately (vulnerable patients, etc.)
dd. Delegate clinical responsibilitiesappropriately (ensuring safe practice and that the task is within the scope ofpractice of the individual)
ee. Support the clinical team with allsafeguarding matters in accordance with local and national policies
ff. Understand practice and local policiesfor substance abuse and addictive behaviour, referring patients appropriately
gg. Deliver opportunistic health promotionwhere appropriate
hh. On a rotational basis, undertake dutyroles. During your assigned duty times you must be onsite and available.

Qualifications
Essential
1. - Registered Nurse with Nursing and Midwifery Council
2. - Masters degree required for qualification post December 2020 refer to RCN Credentialing for Advanced Level of Nursing Practice.
3. - Post graduate diploma or degree for Advanced Practice Qualification up to December 2020
4. - Qualified Independent Nurse Prescriber on the NMC register
5. - Meets NMC revalidation requirements in accordance with the NMC Revalidation booklet
6. - Meets the standards for registered ANP working at advanced level
7. - Minor illness qualification
