The Practice Nurse is to support new-to-practicenurses and other healthcare professionals within the organisation and acrossthe wider network to enhance integration with community, secondary, and socialcare services.
Thisorganisation must ensure that the post-holder has access to appropriateclinical supervision and an appropriate named individual in the organisation toprovide general advice and support on a day-to-day basis. Main duties of the job
* Deliver high-quality nursing care to patients, including wound management, immunisations, cervical screening, and long-term condition reviews (e.g. asthma, COPD, diabetes).
* Support chronic disease management through patient education, treatment plans, and regular health monitoring.
* Undertake health promotion, lifestyle advice, and preventive care such as NHS Health Checks and vaccinations.
* Assist with treatment room duties including ECGs, blood pressure monitoring, ear care, injections, and sample collection.
* Maintain accurate clinical records and ensure care is delivered in line with current evidence-based practice and clinical guidelines.
* Work collaboratively with GPs, healthcare assistants, and the wider multidisciplinary team to provide safe, effective patient care.
* Participate in safeguarding, infection control, medicine management, and contribute to QOF, enhanced services, and practice audits.
* Support patients with complex needs and provide continuity of care where appropriate.

Job responsibilities
The Practice Nurse is to support new-to-practicenurses and other healthcare professionals within the organisation and acrossthe wider network to enhance integration with community, secondary, and socialcare services.
This role is responsible for the delivery of evidence-basednursing services, working as part of the practice multi-disciplinary team,delivering care within their scope of practice to the entitled patientpopulation.
Working autonomously, the PracticeNurse will be responsible for a number of clinical areas such as public healthand screening programmes, including infection prevention and control, chronicdisease management, immunisations, health promotion and prevention, wellman/woman clinics and cervical screening, Furthermore, they embed populationhealth management approaches within general practice, targeting healthinequalities and improving access.
The post-holder will be an integralpart of the general practice team, as well as supporting the wider practicenurse network and multi-disciplinary team. They are to participate in formaltraining events and to promote best practice in their area of expertise as wellas supporting the practice management team in the reviewing and delivery ofclinical policy and procedure.
