What you will do
* Conduct complex person-centred assessments, including those related to mental capacity, to support individuals in achieving the highest level of independence possible.
* Provide expert advice and support to carers in managing challenging situations.
* Accurately record and manage social care data in compliance with organizational policies.
* Engage with multi-disciplinary teams, offering expert opinions and acting as a liaison between professionals.
* Guide and mentor junior staff, providing supervision and support to newly qualified social workers.
* Organize and chair meetings to fulfil legal requirements under the Mental Capacity Act 2005.
* Undertake safeguarding responsibilities and act as an Investigating Officer in accordance with local policies.
* Develop and coordinate support plans in collaboration with individuals, families, and external partners.
