The University Medical Group is a standalone practice and is the largest in West Berkshire with over 32,000 patients. We are based in Reading near the University sites, where we have over 9,000 students as patients. As a PCN in our own right, it's easy to make changes to ensure we do the best for our patients, and being innovative is our priority. We have received outstanding ratings on the last 2 CQC audits and are very proud of this achievement, with quality patient care being our key focus.

We are seeking a full-time Pharmacist who can work until 20:00 one day per week.

Tier 2 sponsorship is not available.


Main duties of the job

* Repeat prescribing
* Conducting face-to-face and telephone appointments with patients
* Chronic disease management
* Reconciliation of medicines following hospital discharge
* Providing telephone support to patients
* Managing care home prescriptions
* Performing clinical audits
* Supporting the prescribing lead to achieve maximum points in the Prescribing Incentive Scheme
* Ensuring patient safety through drug safety alerts and compliance with CQC standards
* Training students
* Managing secondary care prescriptions
* Liaising with local community pharmacists


About us

The University Medical Group is a large multidisciplinary team comprising 3 GP Partners, 15 Salaried GPs, 4 Nurses, 1 Nursing Associate, 3 HCAs, 1 Physician Associate, 3 Paramedics, 5 Pharmacists, and 1 Pharmacy Technician.


Job responsibilities

* Managing the repeat prescribing authorization process, reviewing patient requests, and making necessary changes as an Independent Prescriber
* Ensuring patients are booked for necessary monitoring tests
* Switching to cost-effective medication equivalents
* Promoting the use of Electronic Prescriptions
* Synchronizing prescriptions where necessary
* Linking prescriptions to medical conditions
* Reconciliation of medicines post-discharge, identifying and rectifying unexplained changes without GP referral, performing medication reviews, and arranging follow-up tests
* Providing urgent medication prescriptions and reviews via telephone
* Supporting community pharmacists and nurses with prescription queries
* Reviewing and issuing care home prescriptions as an independent prescriber
* Collaborating with the CCG Medicines Optimization Team and clinical medication reviews in care homes
* Performing ad hoc clinical audits to support the Prescribing Incentive Scheme
* Supporting achievement of maximum points in the Prescribing Incentive Scheme and managing alerts from MHRA and drug safety updates
* Providing training to NHS students or trainees
* Managing requests for new medications from secondary care, ensuring patient understanding and appropriate follow-up
* Facilitating dialogue with local pharmacies for optimal service delivery
* Conducting face-to-face medication reviews, including blood pressure checks, and providing influenza vaccinations based on experience and training


Person Specification


Qualifications

* Registration as a pharmacist with the General Pharmaceutical Council
* Full driving license and access to a vehicle
* Good communication and interpersonal skills
* Membership of the Royal Pharmaceutical Society
* Independent prescribing qualification or willingness to undertake it
* Experience working with multidisciplinary healthcare teams in primary care
* Proactive, dynamic, and able to use own initiative
* IT literate and proficient in computer use
* Ability to teach others
* Strong problem-solving and decision-making skills
* Effective workload management
* High standard of clinical skills with experience in various situations
* Commitment to patient-centered care
* Team player with effective liaising skills with colleagues and external agencies

Starting salary ranges from £23.00 to £28.86 per hour depending on experience.
