Offer description

This position is offered initially as a 12-month fixed-term contract, with the potential for further 12-month extensions and/or the opportunity to progress into a permanent position. There may also be future opportunities for increased hours for candidates who would welcome additional sessions as the role and service develop.

We are looking for a motivated and compassionate Practice Nurse to join our friendly and forward-thinking general practice team for two days per week. This is an exciting opportunity for a nurse who enjoys working autonomously, building strong patient relationships, and making a genuine impact on long-term health outcomes within the local community.

The role offers the chance to lead and shape meaningful chronic disease and quality improvement work, particularly in Chronic Kidney Disease (CKD) and Cardiovascular Disease (CVD), while also maintaining a varied and rewarding clinical workload. You will be supported by an experienced multidisciplinary team who value collaboration, innovation, and high-quality patient care.

This role would particularly suit someone who enjoys structured clinical work, preventative healthcare, and being part of a practice that is committed to continuous improvement and patient-centred care. In return, we offer a welcoming working environment, supportive leadership, opportunities for professional development, and the ability to make a visible difference within a close-knit practice team.
