The Primary Care directorate at ELFT have an exciting opportunity for an enthusiastic, motivated, and innovative Senior Clinical Pharmacist to join our Team.
We are looking for an individual who will support medicines management and be a clinical leader for medicines optimisation within our two surgeries.
Main duties of the job
Main duties:
1. Work as part of a multi-disciplinary team taking responsibility for
2. areas of chronic disease management within the practice
3. Undertake clinical medication reviews to proactively manage
4. patients with complex poly-pharmacy, especially for older
5. people, people in residential care homes and those with multiple co-morbidities
6. Provide primary support to general practice and GPSU staff with regards to prescription and medication queries
7. Clinical leadership of medicines optimisation, plan and organise quality improvement initiatives, appropriate specialist services and manage some aspects of the quality and outcomes framework and enhanced services
8. The role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver excellent service within general practic.

About the Candidate
You will have the following skills/experience:.
9. Professional registration with GPhC
10. Masters degree in pharmacy (or equivalent)
11. Experience of working as a qualified, registered pharmacist in primary care, community or hospital pharmacy
12. Independent prescriber
13. Awareness of general practice and the agenda for medicines optimisation
14. Good clinical pharmacy knowledge including terminology

17. A member of or working towards Faculty membership of the Royal Pharmaceutical Society
Qualifications

18. Master's degree in pharmacy (MPharm) (or equivalent)
19. Specialist knowledge acquired through postgraduate diploma level or equivalent training/experience
20. Independent prescriber

21. In depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare.
22. An appreciation of the nature of GPs and general practices
23. An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving Prescribing
24. Excellent interpersonal, influencing and negotiating skills
25. Excellent written and verbal communication skills
26. Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences ( patients)
27. Is able to plan, manage, monitor, advise and review general medicine optimisation issues in core areas for long term conditions
28. Good IT skills
29. Able to obtain and analyse complex technical information
30. Recognises priorities when problem solving and identifies deviations from the normal pattern and is able to refer to seniors or GPs when appropriate
31. Able to work under pressure and to meet deadlines
32. Produce timely and informative reports
33. Gain acceptance for recommendations and influence/motivate/ persuade the audience to comply with the recommendations/ agreed course of action where there may be significant barriers
