Overview

We are seeking a motivated and dynamic nurse to join our colorectal team as a Colorectal Cancer Pathway Nurse Practitioner. This opportunity aims to facilitate faster diagnosis for patients on a 28 day pathway, providing care and support for patients on suspected cancer pathways. The post requires a nurse with colorectal and/or relevant general surgical experience who is looking to develop their career. The post will require cross site working. Training and support will be provided.

This specialist nursing in post will:

* triage patients referred onto the suspected colorectal cancer pathway (TWR / 28 day pathway)
* support the faster diagnosis and improve outcomes for patients with suspected colorectal cancer by organising appropriate investigations
* review and action on results of investigations
* improve patient experience by ensuring effective communication and being a point of contact within the colorectal team
* support the Colorectal Cancer Pathway Nurse Specialist
* deputise for the Colorectal Advanced Nurse Practitioner in their absence

You will be the point of contact for patients referred onto the colorectal suspected cancer pathway/Two-Week Rule. Main duties of the role are:

* Triage patients referred onto the suspected colorectal cancer pathway (TWR / 28 day pathway)
* Support faster diagnosis and improve outcomes for patients with suspected colorectal cancer by organising appropriate investigations
* Review and action on results of investigations
* Improve patient experience by ensuring effective communication and being a point of contact within the colorectal team
* Attend and participate in multi-disciplinary team meetings and patient tracking list meetings, helping to identify delays in the patient pathway and work towards reducing delays
* Develop effective working relationships with the Surrey and Sussex Cancer Alliance, the CCG and primary care, including data collection for audit purposes to aid service improvement

The successful candidate will work alongside the colorectal team and cancer services to support and facilitate faster diagnosis for patients on a suspected cancer pathway. Full support alongside our Colorectal Nursing team will be offered throughout. The post requires an individual who can work alongside our nursing and medical colleagues in surgery and oncology and also work autonomously once confident to do so. The successful candidate will complete telephone clinics. The post will require cross site working and support tertiary referrals.

You will have a clear understanding of the surgical nursing, cancer pathways, and be committed to improving patient experience. You will also be committed to the development of specialist nursing practice and demonstrate evidence of ongoing professional and academic development. You will be required to complete the necessary training required to order radiological tests.

A successful appointment will depend upon interview and fulfilling service requirements.
