Role background

This role is in partnership with Bradford District Care Trust, with the purpose of supporting physical and mental health of service users living with Severe Mental Illness (SMI) or those with complex needs, through accessing the green and blue spaces across the Bradford District. For reference, green spaces refer to natural areas such as parks, woodlands, and gardens, while blue spaces include environments with water, such as rivers, lakes, and canals. These settings are known to have therapeutic benefits and play a vital role in promoting mental wellbeing, recovery, and social connection.

Purpose of the role

The role is designed to support individuals living with Severe Mental Illness (SMI) through a collaborative approach involving primary and secondary care partners, as well as VCSE services. Working in partnership with Bradford District Care Trust, the postholder will deliver person-centered mental health support within the natural spaces of the Bradford District. By harnessing the therapeutic benefits of nature, the role aims to empower individuals in their recovery journey, promote wellbeing, and connect them with sustainable resources and community support to help them thrive.

Key duties

*
o Provide face-to-face support to service members from a variety of backgrounds that are living with a severe mental illness (SMI) or complex needs.
*
o Deliver nature-based activities for service users, including guided visits to green and blue spaces across the district. Collaborate with local nature-based organisations to create meaningful opportunities that support wellbeing, recovery, and social connection.
*
o Have a good understanding of support services across the district and able to complete onward referrals to relevant services for people to access additional support.
*
o Complete individual needs assessments, risk assessments and outcome measures with people we support.
*
o Develop person-centred support plans to help people stay well.
*
o Record information electronically and maintain up to date records.
*
o Work effectively with colleagues.
*
o Motivate and encourage people to take steps to achieve positive change in their lives through the use of nature.
*
o Promote equity, diversity and inclusion through your work.
*
o Commit to self-development and undertake any training required to fulfil your role.
*
o Participate in individual supervision and internal/external staff development including mandatory training related to the role
*
o Attend and input to regular team meetings as well as whole organisation engagement days
*
o Build effective working relationships with Bradford District Care Trust colleagues and be able to co-work with them in shared working spaces to help drive the service.

Qualifications

Essential:

* Level 3 or above qualification in a relevant field or at least 2 years relevant experience

Experience

Essential:

* Paid or volunteer experience of working with young people or adults in a health, social care or any other settings.
* Experience of supporting people in a clear, meaningful and non-judgemental way in face-to-face settings (this can include voluntary experience).
* Experienced and confident in managing challenging situations, and recognising and responding to safeguarding concerns (this can include voluntary experience).

Skills and Abilities

Essential:

* Excellent proven communication and organisational skills, including an ability to encourage and engage people in ways to make positive changes in their lives.
* Excellent IT skills, including use of Microsoft Packages
* Ability to work as part of a team and work alone and use own initiative
* Ability to gather relevant information to support and assess risks

Knowledge

Essential:

* Understanding of the key issues within mental health, and the potential challenges that people may face
* Knowledge of other relevant health & social care organisations across the Bradford District & Craven
* Knowledge of risk management and health and safety issues
* Understanding of safeguarding and good practice
* Understanding of confidentiality and data protection
