An exciting opportunity has arisen for a Registered Nursing Associate to join the South West Older Adult Community Team. The applicant must have completed the Nursing Associate foundation degree and be a registered Nurse Associate on the NMC register. This post is not offering training to gain this qualification.
The successful applicant will work as part of a busy community team, acting as a keyworker for a defined caseload. Responsibilities include supporting medicines management and psychological interventions, collaborating with patients and families for goal setting, and meeting individual needs.
The role involves identifying the team base aligned to the applicant, with clinical demands covering both the South and West areas of the team's work. You will work within an integrated community mental health team, providing recovery-focused interventions, managing a caseload, and liaising with multi-agency services within Tees Esk and Wear Valleys NHS Trust.
Following initial assessments by the Registered Nurse, you will implement care plans, provide information and advice, and evaluate and adjust care in partnership with patients, carers, and the multidisciplinary team.
You will promote health, deliver high-standard interventions, respond to enquiries, address concerns, and escalate issues as needed. Accurate risk assessments using contemporary tools are essential to ensure safety and quality of care.
