You will work collaboratively within a multidisciplinary team, helping to formulate and implement support plans to foster independence and ensure optimal living arrangements for individuals in the community. You are dedicated to promoting self-directed support and are knowledgeable about welfare benefits and community resources. What the Job Involves Conducting assessments in line with the Care Act and eligibility criteria. Collaborating with health and social care colleagues for integrated service provision. Creating strengths-based support plans to enhance independence. Facilitating service users in commissioning support resources tailored to their needs. Managing a personal caseload with varying complexity, with oversight from senior staff. Carrying out risk assessments and supporting safeguarding interventions. Supporting service users and informal carers with welfare benefits navigation. Participating in the development and implementation of individual support plans and programs. Contributing to service improvement initiatives and participating in team activities. Skills Recent experience in social care environments. Understanding of mental capacity, safeguarding, and equal opportunities legislation. Ability to work collaboratively in a multi-agency setting. Capability to maintain accurate records and use relevant ICT systems. Competence in identifying and implementing appropriate interventions. Excellent communication skills, both verbal and written. Ability to recognize and address communication barriers. Understanding of welfare benefits and self-directed support principles. Adaptability to work with diverse client needs and in challenging environments.
