Overview

We welcome applications from those who currently hold a UK NMC registration. If you currently hold an overseas nursing qualification and require sponsorship we are not currently direct hiring Internationally Qualified Nurses. The Acute Children's Outreach Nursing Service (ACORNS) started in August 2017 in response to increasing admissions of acutely unwell infants and children who could be managed safely in their own homes with the correct support and ongoing assessment. The ACORNS team is part of the Sussex Virtual Ward and Children's Emergency directorate, working closely with services across the Royal Alexandra Children’s Hospital to facilitate continuing assessments of acutely unwell patients. The service aims to provide evidence-based, structured clinical telephone and video assessments, support families at home, and maintain consistent and continuing care across platforms.

Responsibilities

* Provide holistic, autonomous nursing care using evidence-based practice, including initial assessments and home visits, planning, implementing, and evaluating care for patients under continuing responsibility.
* Conduct physical assessment, initiate nursing and emergency interventions, recognise deterioration in a child, and monitor acute exacerbations of illness while aligning practice with current local and national guidelines for the sick child.
* Work in partnership with the child and family to assess needs, plan care, provide advice, and participate in health promotion; communicate across multiple platforms to support families in the home.
* Initiate appropriate work, accept referrals, and work autonomously in community settings; regularly liaise with acute hospital colleagues to enable timely discharge and appropriate follow-up, including complex scenarios such as viral wheeze, diarrhoea with dehydration, and other high-volume conditions seen in the CED.
* Provide structured clinical assessment to empower children, young people and parents to meet acute care needs; participate in the Same Day Emergency Care (SDEC) pilot scheme by supporting the consultant-led service through observations, medication assistance, and facilitating interventions as required.
* Utilise a range of IT programmes and maintain up-to-date practice to ensure efficient delivery of care and effective communication with families.


Qualifications and Requirements

* Current UK NMC registration is required.
* Applications from those with overseas nursing qualifications requiring sponsorship are not currently being direct-hired for Internationally Qualified Nurses.
* Ability to work autonomously in the community and undertake home visits as part of ACORNS; experience with paediatric nursing and remote monitoring is preferred.
* Commitment to safe working practices, including DBS checks where applicable and adherence to local and national guidelines for paediatric care.
