Description Social Worker | 9344 | permanent contract | part-time 18.5 hours per week | £39,862 to £41,771 per annum pro rata | Grade J | Norwich We are offering a unique opportunity for a Palliative Care Social Worker to join a highly skilled, multi-agency team supporting patients with life-limiting illnesses at the Norfolk and Norwich University Hospital. This post is funded by the hospital’s Palliative Care Team (PCT), one of the largest in the country. Key Details: Location: Norfolk and Norwich University Hospital (on-site during working hours) Referral Sources: Primarily from the PCT, with additional referrals from other hospital professionals. You will also have scope to identify needs during daily team meetings. Development Opportunities: Enhance your expertise in providing psychological and practical support to patients with serious illness. Access to a psychologist-led support group and potential training opportunities. Core Responsibilities: Support patients navigating the challenges of serious illness. Provide advice and signposting on welfare rights and legal matters. Assist patients with caring responsibilities for family members. Facilitate complex discharges and coordinate with hospital discharge pathways.
