Job Description
We are currently seeking an experienced Band 7 Mental Health Practitioner (MHP) to join the Mental Health Integrated Community Service (MHICS) in Leatherhead on a part-time, ongoing agency basis. This is an excellent opportunity to work within a dynamic team delivering person-centred care in a primary care setting.
This role is suitable for Registered Mental Health Nurses (RMN), Occupational Therapists (OT), or Social Workers (SW) with relevant NHS experience and a solid understanding of community mental health pathways. Key Responsibilities
* Conduct comprehensive mental health assessments and collaboratively develop care plans with service users.
* Work closely with GPs and other primary care staff to provide integrated mental health support.
* Monitor mental state, manage clinical risks, and support treatment adherence, including medication monitoring where appropriate.
* Provide short-term interventions and facilitate referrals or signposting to relevant services.
* Maintain accurate and timely clinical records using SystemOne and EMIS.
Essential Requirements
* Current professional registration with NMC, HCPC, or Social Work England.
* Recent NHS experience in a Band 6 or Band 7 mental health role.
* Proficiency in using SystemOne and EMIS systems.
* Solid understanding of primary care and integrated community mental health services.
* Strong communication, assessment, and risk management skills.
