Core Purpose:
Provide senior oversight and specialist assessment for Urgent Care Services across all PCFT sites.
Ensure purposeful admission and explore least restrictive alternatives for all patients.
Key Duties:
* Gatekeeping & MDTs:
*
o Conduct senior gatekeeping reviews.
o Convene early multidisciplinary team (MDT) meetings when admission outcomes are challenged.
* Clinical Leadership & Advice:
o Offer guidance to teams on acute care pathway assessments.
o Support professional development for staff involved in patient assessments.
* System Alignment:
o Build effective relationships with Acute Providers, Local Authorities, Police, Housing, VCSE, and Commissioners.
o Promote positive risk-taking and least restrictive options.
* Monitoring & Reporting:
o Ensure robust processes for tracking purposeful admissions and gatekeeping outcomes.
o Report professionally to the Consultant Clinical Lead for Patient Flow and managerially to the Deputy Chief Operating Officer.
Team Context:
* Operates within the Trust Wide Patient Flow Team.
Main duties of the job
* Provide visible clinical leadership across the PCFT footprint Emergency Departments.
* Report updates via a monthly progress report to the Trust Integrated Operational and Quality Oversight Group.
* Interface with acute care colleagues, attend relevant meetings, and ensure effective communication processes are in place.
* Complete face-to-face assessments and role model a Community First approach, promoting least restrictive alternatives and purposeful admission.
* Act in a coaching capacity to develop clinical skills and confidence in acute care pathway assessment, supporting positive risk-taking and constructive professional challenge.
* Lead on local service development and change using evidence-based Quality Improvement (QI) methodologies to support patient safety and patient-centred care.
* Engage locally and nationally to appraise best practice models and collaborate with trust teams to implement these.
* Lead or contribute to service reviews and other specified projects.
* Lead and chair complex service user reviews to resolve issues relating to acute pathway flow.
* Attend liaison and home treatment team steering groups to provide an overview of KPIs, development plans, and risk.
* Develop a set of key performance indicators (KPIs) for Home Treatment Teams to triangulate admission and gatekeeping information with community and inpatient quality data.

Detailed job description and main responsibilities
* Prepare reports and papers for the Deputy Chief Operating Officer for presentation to Trust boards.
* Lead and attend meetings (internal and external) on patient flow, service development, and performance; ensure delivery of agreed actions.
* Deliver presentations to internal and external stakeholders, explaining complex information clearly.
* Represent the Trust at forums with commissioners, Local Authorities, voluntary and private sector organizations, promoting a positive image.
* Ensure compliance with Trust policies, procedures, and health & safety requirements for self and others.
* Participate in Silver on-call rota as required.
* Provide cross-cover for the Trustwide Senior Service Manager for Patient Flow when needed.
