St Helens Borough Council, Whiston. Salary: £47,181 to £49,282 per annum. Permanent position.

Making A Difference as an Assistant Team Manager in Contact Cares

An opportunity has arisen for a driven and innovative Assistant Team Manager to join Integrated Health and Social Care in the Front Door Service within Contact Cares at Whiston Hospital and Nightingale House. This large and busy multi‑disciplinary team is the single point of access for health and social care referrals.

As the team continues to develop, we are looking to recruit a motivated Assistant Team Manager to join our service. The team consists of both qualified and unqualified Social Care, Nursing and Therapy staff. We offer planned and urgent assessments and provide a 2‑hour response under Urgent Community Response Criteria.

The Contact Cares team also provides initial contact, care planning, onward referrals and signposting services for a wide range of adult clients across health and social care. With a focus that stretches from prevention and early intervention to reducing hospital admissions and readmissions, the work is varied and interesting.

The team operates from 8.00am until 10.00pm, 7 days a week including bank holidays. This post will be shift working to a rota over 7 days.
