Responsible for planning, delivering and evaluating the personalised health and wellbeing care needs of patients, in line with the Trust’s Values & Nursing & Midwifery Professional Framework goals example: personal comfort, patient flow, vital signs and NEWS, safe transportation, ECG, wound dressings, removal of sutures/clips, venepuncture, palliative and terminal care, admission and discharge, Medication support, psychological and spiritual support, the list is not exhaustive To provide clinical supervision and advice for other staff and students to support them in reaching their full potential.

To support the productive operation of the clinic environment by undertaking a range of activities which contribute to the maintenance of a safe and clean environment.

To act at all times in a manner that upholds the Trust’s values and & Nursing & Midwifery Professional Framework goals, working as part of a team to ensure that patients and relatives receive excellent care with compassion.

Main duties of the job

Assist and support the Dept sister in the safe, effective management of the area including management of personnel, facilities and the associated systems and processes

· Provide guidance and direction to junior staff members and students, monitoring and maintaining standards of care and professional practice

· Act as a role model, presenting a positive image of self, team and the organisation

· To participate in education and training of junior staff

You will work in the Main Outpatient suites covering 17 specialities, Chest clinic and other satellite outpatient areas, but may also at times be also asked to cross site cover to support Chorley Outpatients Department too.

Detailed job description and main responsibilities

Please refer to attached JD for further guidance regards this role - this list is not exhaustive and can be added to as roles progress

·Assess, plan, implement and evaluate care to meet patients’ needs in accordance with agreed local and national standards and evidence based guidelines. Perform Moving and Handling techniques on a daily basis, use of hoist, slides and pat sheets

·Support and provide patient education and health promotion

·Be responsible for the delivery and co-ordination of care, through appropriate professional practice, delegation and supervision of duties carried out by junior staff

·Proactively manage risk and maintain patient safety, conforming to health, safety and security legislation, policies, procedures and guidelines.

·Assist in setting, monitoring and maintaining standards of care to improve the patient experience and outcome of care.

·Communicate all relevant information to the multidisciplinary team internally and with external organisations as required, to ensure seamless care

·Maintain accurate and complete records of all care activities, in a timely manner, adhering to local and professional standards for documentation

·Act as a role model, presenting a positive image of self, team and the organisation

·Promote equality in care and practice by recognising, respecting and meeting the needs and choices of individuals

·Ensures environment of care is safe, well equipped and compliant with standards for hygiene and cleanliness.

·To participate in education and training of junior staff as well as completing appraisals for junior staff

·Maintain and develop own professional competence and seek to extend the scope of personal knowledge and skills within scope of role and professional practice

·Actively try to resolve issues and complaints at local level and then report the outcome to line manager.

2. Provide good quality personalised care in accordance with agreed local and national standards.

3. Take samples from patients for testing or dispatching eg: Urine, sputum, faeces, MRSA swabs, blood sugars, venous blood sampling and ECG recordings - this is not exhaustive

Qualifications & Education

Essential criteria

4. Registered nurse with appropriate NMC Registration
5. NMC recognised Mentorship qualification or willingness to work towards following 18 months commencement in post

7. Awareness of changes within Nursing practice
8. Knowledge of Clinical Governance and the relevance to practice
9. Knowledge of relevant policies and procedures

11. Evidence of clinical competence in Dermatology including, phototherapy, patch testing and iontophoresis
12. Knowledge of Leadership and Management
