The role involves carrying out a range of assessments, including community care support, specialist equipment, minor adaptations, and reablement interventions. It also requires providing advice and guidance on available services, support options, and welfare benefits, helping individuals maximise their wellbeing and maintain control over their lives.
As part of a multi‑disciplinary and collaborative environment, the Social Care Assessor (Sensory) will contribute to safeguarding responsibilities, participate in multi‑agency meetings, and maintain accurate case records using council systems.
About You
The ideal candidate will have demonstrable experience within adult social care, health, or a related field, with a strong understanding of the needs of adults with sensory impairments. Experience of working with a range of client groups, including people with dementia, mental health needs, autism, learning disabilities, and physical disabilities, would be advantageous.
You will be committed to delivering excellent customer‑focused services and have experience of working directly with service users and carers, promoting their involvement in assessment, support planning, and decision‑making. Experience of advocacy, partnership working, and multi‑agency collaboration is essential, alongside the ability to build positive and effective relationships with professionals across health, social care, and community settings.
The successful candidate will be a dynamic and resilient practitioner who can work effectively under pressure, manage competing priorities, and meet deadlines while maintaining high standards of service delivery. Strong communication skills, both written and verbal, are essential, together with the ability to produce clear assessments, support plans, reviews, reports, and case records. The ability to communicate effectively with people who have sensory impairments, including those who are deaf or blind, is highly desirable.
You will be skilled in carrying out strengths‑based, person‑centred assessments and risk assessments, using professional judgement to identify appropriate solutions that promote independence, wellbeing, and positive outcomes. An understanding of preventative approaches and early intervention will support your success in this role.
Candidates should hold a relevant qualification or be able to demonstrate equivalent experience, together with a commitment to ongoing learning and professional development. The ability to speak a language commonly used within Lutons diverse communities would be welcomed.
