The Home First service brings together nurses, paramedics, therapists, health and therapy assistants to work with the acute Multi-Disciplinary Team & Transfer of Care Hubs, promoting efficient and effective discharge from hospital to the patient's own home.
Our multidisciplinary teams work flexibly to provide a 7-day service, 365 days a year, between 8am and 8pm. Patients discharged into Home First receive step-down intermediate care, involving community-based assessments and interventions in their own homes to support recovery and monitor their health post-discharge, aiming to prevent re-admission. The service also offers acute reablement for up to 10 days, supporting individuals to achieve functional goals and work towards independence in daily activities.
The core principle of the service is to facilitate recovery by working towards maximum independence through enablement.

Main duties of the job
As a Health and Therapy Assistant, you will provide personal, health, and therapy care for individuals across Sussex with diverse and acute needs. Your role will be varied and engaging, supporting people to enhance their independence.
You will assist multidisciplinary colleagues in ongoing assessments and care provision, including simple clinical procedures such as taking physical observations, venepuncture, administering medications, wound care, rehabilitation programs, and equipment provision. Observing changes in patients' conditions and maintaining accurate records are also key responsibilities.
No prior experience is required as training will be provided on the job, including basic nursing and therapy skills, supporting you to achieve the Care Certificate. Our comprehensive competency framework will support your ongoing development.
With experience and further training, there are opportunities to advance your career in healthcare, such as training as a Nursing Associate or progressing to degree-level healthcare professions like Nurse, Podiatrist, Midwife, or Therapist.
