Job Introduction
As a Band 7 Physiotherapist, you’ll be part of our Early Supported Discharge (ESD) team primarily based at Gravesham Community Hospital, Gravesend (DA11 0DG).
We have an exciting opportunity for an experienced and motivated Band 7 Physiotherapist to join our friendly Early Supported Discharge (ESD) multidisciplinary team based at Gravesham Community Hospital in Gravesend.
You will play a key role in delivering specialist physiotherapy assessment, intervention, and rehabilitation for patients with neurological conditions, including stroke, supporting safe and timely discharge from hospital into the community.
You will also provide leadership and clinical support within the therapy team, contributing to service development, supervision of staff, and high‑quality patient outcomes.
This is a full time position (37.5 hours per week).
Main Responsibilities
* Work autonomously with a specialist and often complex caseload within an Early Supported Discharge service
* Assess, diagnose, and develop individualised treatment and rehabilitation programmes in collaboration with patients and carers
* Provide specialist neurological physiotherapy assessment and intervention, including stroke rehabilitation
* Use evidence-based practice and outcome measures to demonstrate clinical effectiveness and inform treatment planning
* Deliver clear, sensitive communication to patients and carers regarding diagnosis, prognosis, and rehabilitation goals
* Contribute to safe and timely discharge planning, ensuring appropriate onward referral and community support
* Liaise with multidisciplinary colleagues and external agencies (e.g. GPs, social services, acute hospital teams) to ensure continuity of care
* Provide leadership, supervision, and support to junior staff and students within the ESD team
* Participate in service development, audit, and quality improvement initiatives
* Ensure appropriate equipment provision and referral for specialist assessments where required

The Ideal Candidate
The ideal candidate will have:
* A recognised Physiotherapy Degree/Qualification
* HCPC registration
* Membership of the Chartered Society of Physiotherapy
* Significant post‑registration experience, including at least Band 6 level experience within community or neuro‑rehabilitation settings
* Strong experience working with neurological conditions, including stroke rehabilitation
* Experience working within community-based or Early Supported Discharge services (desirable)
* Evidence of leadership or supervisory experience within a clinical team
* Strong understanding of clinical governance, audit, and evidence-based practice
* Commitment to continuing professional development with demonstrable outcomes
* Excellent communication, clinical reasoning, and organisational skills
* Full UK driving licence with access to a vehicle for work purposes (essential)
