We are seeking a dedicated and compassionate Health Care Assistant to join our Home Visiting Team as part of our Primary Care Network.
The successful candidate will support healthcare professionals in delivering high-quality care to patients in their homes through various tasks, including observations and blood sample collection.

Conduct regular observations of patients, including monitoring vital signs, temperature, pulse, and respiration rates.
- Report any changes in patients' health status to the healthcare team and record on patients notes.
Blood Sample Collection:
- Perform venipuncture and capillary blood sampling as required.
- Patient Care:
- Discuss Future Care plans, support comprehensive geriatric plans, and personalised care plans.
Maintain accurate records of patient observations and care delivered.
- Ensure compliance with health and safety guidelines and organisational policies.
Work closely with the Non-Clinical Home Visiting Service Lead and other healthcare professionals within the service and Primary Care Network to coordinate patient care.
- Participate in team meetings and training as required.

Relevant certification or training in health care support (e.g., NVQ Level 2/3 in Health and Social Care).
Previous experience as a Health Care Assistant or in a similar role is preferred.
Valid driver's license and access to a vehicle for home visits.
